1841 Broadway, Suite 505 (MAP IT!). It is next to the Time Warner Center at Columbus Circle on the corner of 60th and Broadway.

If you need to reschedule your
appointment, please give 24 hours
notice or you will be charged the
full fee of the appointment missed.
